Our Services
Referrals for home care may be made by clients and family members. A physician's order will be obtained to initiate care. Upon referral from a physician, a nurse will perform an assessment of the client in his or her home. A plan of care will be formulated from this assessment. The nurse then coordinates with the home health team to provide quality home care services that ensure specialized attention to the client’s individual needs.
Family participation is important. The agency’s team works to ensure that the client receives the best care possible.
